EXAMPLE PLEDGE CARD

Contribution
Employee selects either to give a one-time
donation or through their paycheck.

Selecting
Employees can select up to 3
charities to support or can opt to have their donation divided between
all the charities.

Employee Information
Required information is determined by
company’s Payroll/Human Resource department. Community Shares and its member
organizations use your information only to send thank you letters and
informational materials. You can opt to remain anonymous if you wish.

Copies
One copy of the pledge form goes to the
company’s Payroll/Human Resources department. The second copy goes to
Community Shares. The final copy goes to the employee to serve as a
receipt for their donation.

List
The back of every pledge form lists
organizations to donate to through the workplace fundraising campaign.
